Service Introduction
NT East Community Rehabilitation Day Center (CRDC) provides time-defined professional rehabilitation service to discharged patients with disabilities in physical and/or cognitive functions with neurological, orthopedic conditions as well as chronic pain management.. It aims at enhancing the patients’ physical functioning and self-maintenance abilities, strengthening their domestic living and community living skills, helping them restructure a healthy lifestyle and productive life roles so as to facilitate their integration into the Community. CRDC also provides training programs and educational courses for their family members / carers to strengthen their caring capacities and relieve their pressure so as to improve their quality of living.
Services
To provide Center-based service for the discharged patients with:
- individual or group rehabilitation training programs
- Training programs/ educational courses/ workshops for the family members/ carers
- Supportive services including the organization of mutual support groups for discharged patients, the launching of public education programs and the provision of consultation services to support groups and rehabilitation units in the community
- Day respite care service
- Day care service for severely disabled persons

Users and Details
- Rehabilitation Training Service
- aged 15 or above; live in NT East Region and suffering from stroke, neurological or physical impairments, or other physical illnesses affecting their functional performance and in need of continuous rehabilitation training.
- Carer Support Service
- Family members/ carers of discharged patients as stipulated above
- Day Respite Service
- discharged patients as stipulated above
- Day Care Service for Severely Disabled Persons
- Severely mentally handicapped / severely physically handicapped persons aged between 15 or 59.
Professional team
The Centre’s professional team comprises occupational therapists, physiotherapists, nurses, social workers and rehab trainers.
Application
- Rehabilitation Training Service
- Referred by the Hospital Authority’s medical officers/occupational therapists/physiotherapists
- Day respite service for the disabled
- Referred by social workers; or
- Direct application to the Centre by family members/carers
- Day care service for the severely disabled
- Referred by social workers
- Carer Support Service
- Referred by social workers or medical personnel;or
- Direct application to the Centre by family members/carers
