Day Care Service
Service Objectives
We provide a package of home-based support services encompassing personal care, nursing care and rehabilitation training, to enable persons with severe disabilities to continue living in the community and enhance their quality of life, as well as to relieve their family members/carers of the stress of care.
Target service users
Persons with severe intellectual or physical disabilities, aged between 15 to 59
Services
- Day care, including a meal
- Nursing care and intensive personal care, including assistance in daily activities
- Regular social and recreational activities to encourage social engagement of service users
Operation Hours
Monday to Friday 8:00am to 6:00pm
Fees
Day-care service: $1,002 per month (lunch included), or $61 per day
Application
Referrals can be made by social workers directly to a specific service unit. Application is free of charge.
Home Respite Service
We provide a home respite service for people with severe disabilities to help relieve pressure for home carers. Our professionally trained caregivers can make home visits at appointed hours to take care of the disabled at their homes.
The respite service is available from 8.30am to 6.30pm on Monday to Saturday. Care for extended daytime and holidays can be arranged as needed (overnight support is not available).
Case social workers will work out care plans with the service user and/or the carer, covering the service user’s daily routines such as personal care (bathing, toileting, medication, etc.), meal arrangements, training and outdoor activities.
Respite Service
We provide a respite service for people with tetraplegia to live away from home temporarily in order to give their home carers a breather to handle personal affairs or take a short break to relieve pressure.
Target service users
The service is offered to people who are tetraplegic due to spinal cord injuries, neuro-muscular diseases or severe spasticity, or people with severe physical disabilities who rely on respiratory machines to breathe. Applicants must be above 15 years old, living at home, medically and mentally stable, not suffering from infectious diseases and having a concrete discharge plan.
Service duration
Applicants can apply for the service for multiple times in a year, for a total of no more than 30 days within a calendar year.
Application
Please submit your application form 30 days before requiring the service. The centre will meet with every applicant to assess if the applicant can be admitted for the respite service. Please download the application form via the centre’s website or pick up from the centre.
